A territorial reorganization of the circumscriptions of the Prelature of Opus Dei has been in progress since 2021, as Msgr. Fernando Ocáriz explained in a message on 30 January 2021.
The current circumscriptions (as of 9 November 2023) are listed below, including the countries or zones within their respective territories. The criterion followed is that established in the decrees by which each circumscription has been erected. The list is arranged alphabetically.

- Central America: Costa Rica, El Salvador, Guatemala, Honduras, Nicaragua and Panama.
- Central Europe: Austria, Germany, Hungary, Romania and Switzerland
